WKC HEALTH CLINIC
SERVICES
CERF Clinic
with Dr. Nancy Bromberg, Veterinary Ophthalmologist.
The CERF exam is a general eye exam, which screens for genetic and
non-genetic abnormalities in dogs that are being considered for a
breeding program. The CERF exam is not a medical examination for
existing problems.
Cardio Clinic
with Dr. Michael S. Miller, M.S., VMD, Dip, A.B.V.P.
The cardiac exam includes an echocardiogram, ultrasound, ECG,
auscultation, and a brief report for your veterinarian. The cardiac exam
screens for congenital heart defects, Cardiomyopathy, valvular disease,
and arrhythmias.

AKC DNA Certification
The DNA Profile Program is for owners and breeders electing to add
value to their breeding programs by eliminating concerns or questions
about identification and parentage. AKC DNA Profiling is required for:
Frequently
Used Sires (dogs having the greatest impact on the AKC Stud Book)
Imported
breeding stock
Dogs
whose semen is collected for fresh-extended or frozen use
The
sires, dam and puppies for Multiple-Sired Litter Registration.
We will collect the DNA sample from the dog and return it to AKC.
You will receive a certificate directly from AKC in about six weeks.

The ICSB service is quick and efficient. In less than 1 hour,
collection and freezing is complete.
Tattoo-A-Pet
with Joseph Quinn.
Joe Quinn is an authorized agent for Tattoo-A-Pet. Tattoos are
placed on the inner thigh, where they are easily visible if your dog
should be lost or stolen.
AKC Canine Good Citizen Test
An AKC certified CGC evaluator will be on hand to administer a
limited number of CGC tests. The test is open to all purebred and mixed
breed dogs. CGC is a certification program that is designed to reward
dogs who have good manners at home and in the community. CGC provides an
excellent foundation for all other training.

IMPORTANT INFORMATION
REGISTRATION:
Registration
form return deadline is FEBRUARY 1, 2008. NO appointments will be
scheduled after the deadline--No exceptions.
Please
arrive at the clinic 15 minutes prior to your scheduled time.

SCHEDULING
Remember
to indicate your time preference on the registration form. We will
do our best to accommodate your requests.
You
may be scheduled in a time frame earlier than the one that you
requested if enrollment in the selected clinic is low.
Appointments
and time preferences will be scheduled on a first come first serve
basis.
You
may be scheduled in a time frame later than the one that you
requested if enrollment in the selected clinic is high.
If
you register for multiple services, every effort will be made to
cluster your appointments.
You
will be notified of your appointment time approximately one week
prior to the event by email or phone if email is unavailable.
REFUNDS
There
will be NO Refunds after the closing date except when clinics are
cancelled by the WKC due to low enrollment.
DOCUMENTATION
Remember to bring necessary paperwork.
Rabies and Licenses
You
must bring the dog’s current/previous rabies certificate.
Bloodwork:
Bring
a self addressed stamped envelope so that your bloodwork results can
be mailed to you.
If
you do not provide an envelope, a $1.00 shipping and handling fee
will be added to your total.
Please
fill out a separate bloodwork submission form for each dog.
OFA Thyroid Database Application
You
can download and complete your OFA Application for Thyroid Database
at

Bring
the completed form to your appointment on the day of the clinic, so
that the veterinarian can complete his/her section.
The
OFA will send a certificate directly to the owner in four (4) to six
(6) weeks.
If
you intend to fill out your OFA Application at the clinic, ensure
that you bring the information listed below to your appointment on
the day of the clinic (See Information About Your Dog below).
Microchip registration requires:
Your
vet’s name and contact information
A
secondary contact person’s name and phone number.
If you would like the WKC Health Clinic Coordinator to mail the CERF
forms to you ahead of time, you must:
Enclose
one self-addressed stamped business sized envelope for each CERF
form with your registration; OR,
One
envelope of sufficient size and postage to accommodate the number of
forms that you request.
Information About Your Dog
DNA Certification. OFA Thyroid Application, and CERF registration
require that you bring the following information to your appointment on
the day of the clinic:
